2015 HEARTLAND HEROES NOMINATION FORM

The American Red Cross needs your help in identifying and recognizing heroes in the Greater Decatur area. Every day people do heroic acts that make our community a better place to live. These humble heroes collect food items for the local pantry, shovel sidewalks for elderly neighbors or use their CPR training to save a life. The Red Cross is seeking nominations for the 2015 Heartland Heroes – a special event to recognize and honor our local heroes. Nominations are being accepted in four categories: Youth, Armed Forces & Public Service, Good Citizen and Red Cross Volunteer. If you know a local hero, please take a few minutes to complete the Heartland Heroes nomination form and tell their story. The nomination deadline is February 13, 2015. Nomination forms are available by emailing pam.tish@redcross.org, at the Red Cross Office at 2674 North Main Street, Decatur, or by calling 217-428-7758, ext. 7478.

Winners will be recognized at the Heartland Heroes event on Saturday, March 7, 2015 at the Decatur Conference Center & Hotel. The evening will include dinner, presentation of the Heartland Heroes
Awards, a silent auction and a special performance by country music legend Joe Diffie.
Tickets are $75 and include cocktail hour, dinner, awards ceremony and the concert – tables of 10 are available for $750. Doors open at 5:30 p.m. with a cash bar available. The dinner will begin at 6:30 p.m.,
the awards ceremony at 7:00 p.m. and the concert will start at 8:00 p.m. Concert Only tickets are available for $25 each and doors for the concert will open at 7:30 p.m. You can purchase tickets online at www.redcross.org/il/decatur or by calling 217-428-7758, ext. 7478.
